Application Process

Applications for admission to Long Term Care facilities are handled through the Community Care Access Centre (CCAC). Our closest office is Owen Sound. If an applicant is currently in a hospital, then his or her application will be handled by the hospital staff and a CCAC Case Manager. If the applicant is not in a hospital, then his or her application will be handled by a CCAC Case Manager. The Case Manager will meet with the client and family and gather the information required to determine if the client is eligible for admission. A Placement Coordinator may assist the applicant in selecting a facility and answer any questions the applicant might have. Applicants who are not currently residing in Grey or Bruce County should contact their local CCAC to initiate placement at Golden Dawn.
